Jacky Lagrace is Vice President of Population Health for Ascension Florida and Gulf Coast. In this role, Lagrace is responsible for $1 billion in accountable care for Ascension communities served in Florida and the Gulf Coast.  The Creator positioned me to lead with mission driven leaders in the Midwest, Florida and California and I’m thankful for the community driven leaders who helped influence my path in healthcare.  Jacky’s background is in clinically integrated networks, population health management, and value-based care partnerships.  Jacky is passionate about working with physicians, practice administrators, care teams, and health system leaders to identify and advance priorities in population health, clinical practice efficiency, processes and outcomes.  Previous roles for Jacky include Executive Director for Baptist Physician Partners in Jacksonville, Florida and Executive Director at Adventist Health in northern California.  Jacky obtained a Master of Health Services Administration from the University of Michigan and a Bachelor of Health Sciences from the University of Florida with high honors.  Additionally, at UF Lagrace received the College of Public Health and Health Professions’ Outstanding Student Award.  Lagrace who is a national speaker on strategic interventions towards advancing value-based care and population health is the author of “Patients Should Take Active Role in Health with PCPs” and is an advocate for helping people especially children and the elderly achieve optimal health.  Lagrace also served on the Board for WeCareJax, an organization in Duval County that coordinates compassionate health care services for people who are uninsured.
